For your convenience, I use a health record called Simple Practice. Once an appointment is scheduled, I will send you a link to your private portal to complete paperwork online.
Important Fee Information
Services may be covered in full or in part by your insurance plan. However, not everyone is comfortable going this route and may opt to pay out of pocket for their therapy services. If you opt to utilize insurance, please contact your health insurance provider to verify coverage, amount of sessions covered, deductible and copay or coinsurance amounts. I currently accept cash, check, or major credit and debit cards.
Initial Intake: $160
An initial 50 minute intake session will include the collection and documentation of comprehensive history, potential diagnosis, goal setting, and treatment planning.
Individual Session: $135
$100 for missed appointments/late cancellations.
Counseling sessions are typically 50 minutes. You can request more time in advance. Please talk to me about fees for extended sessions.
I accept BCBS and Humana insurance plans. When using insurance, here are some questions to ask your insurance provider prior to your appointment:

How much do you pay for an out-of-network provider?

Is there a limitation on how much you will pay per session?

Is there a reference number you can get from the representative in case you have issues with eligibility?
FYI, if you are under someone's insurance plan (spouse or parent), I will need their date of birth for billing.
I am also considered an out of network provider, if your plans offers this type of benefit. I will provide you with a receipt to submit to your insurance company for reimbursement. (Please inquire directly about your out of network benefits with your insurance company).
